Free Model Release Form for Adult Creators

A model release is the written, signed permission a producer gets from each performer before publishing content they appear in. It documents the three things that matter if anyone ever questions a scene: the performer is of legal age, they agreed to be on camera voluntarily, and they granted the producer the right to distribute the result. Platforms increasingly want a release on file before they'll publish collab content — and the platform's own upload form doesn't replace it. That form protects the platform; this one protects you. This release covers a single performer and a single production. For ongoing collaborations or anything with shared ownership, use the model agreement. For the producer-side identity records federal law requires, use the §2257 form tool.
Frequently asked
- Do I need a model release for adult content?
- Yes. Producers need written, signed permission from each performer. Some platforms also require a release on file before they'll publish.
- Single performer or multi-performer?
- This release covers a single performer and a single production. For longer collaborations or repeated uploads, use the model agreement.
- Does this satisfy §2257?
- No. §2257 is a separate record-keeping requirement. Use the §2257 form tool for that.
- Is anything I sign here stored on the site?
- No. The release runs entirely client-side. The signed PDF lives only on the device that generated it.
